Change the actual box, your cat may be smelling something in the box. If that doesn't work, or if there's blood in the stool/urine, your cat may either have a UTI or parasites and the litter is to harsh for him/her to use and your carpet or hardwood floors feel better to go on. Take it to the vet and see if he's sick, your vet will give him some medicine (I forget what it's called). Even though it starts immediately, you still have to give it to your cat for at least three days.
Probably defiance or you're not cleaning it enough. If there has been a sudden change in the house - new animal, new baby, you're away a lot....etc. those things piss of kitties something fierce and they take it out on you by peeing everywhere else.





Good luck fixing the bad habit, it's a hard one to break...crates and locking them in with the litter box helps.
